Award Nomination Form: Albert Hanzlick Paraclete Award

Jennings’ "Albert Hanzlick Paraclete Award" is given annually to one or two Jennings staff members who exemplify the mission and values of Jennings and have provided exemplary service for five or more years. These staff members are nominated by their peers, managers or residents and are presented with a monetary award of $500 at our annual Christmas Party.
The criteria for this award is as follows:

• at least five years of employment at Jennings
• may be retired
• excellent attendance
• demonstrated consistent display of values
• excellent performance evaluations
• no current corrective action on file
• not previously awarded the Paraclete Award
• available to any staff member regardless of position

Nominations must be submitted to Human Resources by Monday, November 18, 2024 using this form or our printed form. Past recipients of the Paraclete Award are available by contacting the Human Resources Department at ext. 2261.
